Code of Conduct & Child-Safety Acknowledgment
WayPath gatherings should be safe, respectful, faith-rooted, educational, and worthy of family trust.
1. Respect and care. I will treat students, families, coworkers, hosts, and partners with dignity. I will not engage in harassment, bullying, discrimination, humiliation, retaliation, threats, or inappropriate language or conduct.
2. Appropriate boundaries. I will maintain age-appropriate physical, emotional, digital, and relational boundaries. I will avoid private or secret communications with minors and follow WayPath’s communication and supervision expectations.
3. Supervision and safety. I will follow approved check-in, release, emergency, transportation, allergy, medication, tool, equipment, and facility procedures. I will not place a student in an unsafe or unauthorized situation.
4. No abuse or exploitation. Abuse, grooming, sexual conduct, sexualized conversation, exploitation, hazing, corporal punishment, and inappropriate images or messages are prohibited.
5. Reporting concerns. I will promptly report suspected abuse, neglect, unsafe conduct, injuries, threats, policy violations, or boundary concerns to the appropriate WayPath leader and, when required, the proper authorities. I understand internal reporting does not replace legally required reporting.
6. Privacy and records. I will protect student and family information, photographs, educational records, medical information, and contact information. I will use information only for authorized WayPath purposes.
7. Faith and representation. I will not misrepresent WayPath, LifeLabs Learning Collective LLC, AliYah Academy, scholarship organizations, partners, credentials, program approval, or my authority. I will communicate graciously with families from different stages of faith and understanding.
8. Compliance. I will follow applicable law, facility rules, insurance requirements, approved WayPath policies, and role-specific instructions. I understand participation may be limited or ended when safety, trust, conduct, or program needs require it.
I have read, understand, and agree to follow this Code of Conduct and Child-Safety Acknowledgment. I understand that concerns may be reviewed and that participation may be suspended during review.
Background Screening & Disclosure Acknowledgment
This document records the participant’s agreement to provide truthful disclosures and complete any screening required for the role. A screening company’s legally compliant authorization and disclosure forms must be used when applicable.
Answering “yes” does not automatically disqualify a person. WayPath may consider relevance, time, circumstances, rehabilitation, legal limitations, safety, insurance, facility rules, and role responsibilities.
Do not collect Social Security numbers, birth dates, driver-license numbers, or detailed criminal-history information on this public form. Use a secure screening provider and attorney-reviewed process.
